And although the total energy of radio waves around us has increased by more than a million times since the beginning of the 20th century, our body does not perceive them. Compared to light, which can cause burns or long-term photoaging, radio waves are much safer and also imperceptible. Therefore, in aesthetic medicine, radiofrequency technologies for influencing the skin are gaining more and more popularity, which, compared to laser technologies, are less aggressive and therefore safer, practically do not cause complications and do not require a long rehabilitation period. We will talk about one of the recently appeared methods of structural and visual skin rejuvenation - fractional radiofrequency rejuvenation - in this article.

The principle of fractional radiofrequency rejuvenation

The procedure of skin rejuvenation can be compared with the renovation of an apartment, when it is necessary to remove unnecessary garbage and rubbish, vigorously renovate the walls and at the same time not create inconvenience to the inhabitants of the dwelling (or create them for a minimum period). Translating this into the language of aesthetic medicine, we must “melt” the old collagen in the dermal layer, stimulate cells to proliferate and synthesize, but at the same time, as far as possible, do not injure the epidermis too much (the “roof” of our skin). So, first of all, it is necessary to deliver energy to the skin matrix in order to destroy the “old” matrix proteins, which, due to “age-related” modifications (oxidation, intramolecular cross-linking, glycation), become resistant to proteases. In laser technologies, the “disintegration” of proteins occurs when the tissue is strongly heated as a result of the absorption of light energy by chromophores. Getting the laser beams to pass through the epidermis without affecting it is not so easy - it's like trying to shout through a loudspeaker to a distant neighbor, but so that the rest do not hear it. On a mobile phone using radio waves, this task is not a problem. So here - using the appropriate radio frequencies tuned for absorption by the components of the dermis, it is possible to deliver energy sufficient to melt collagen without affecting the epidermis. This is especially important in the case of dark skin, the epidermal pigments of which absorb light, but not radio waves, in a wide range.

Radio frequency (RF) energy- a form of electromagnetic energy in the frequency range from 300 MHz to 3 kHz. Absorption in the radio frequency range occurs due to a rapid change in the polarity of the electric field, in which charged molecules such as ions, low molecular weight polar compounds, proteins, phospholipids with polar heads, nucleic acids begin to reorient. Since the aforementioned charged molecules have different sizes, some molecules have time to "follow" the change in polarity, while others, larger ones, do not. Therefore, depending on the frequency of the field, it is possible to spin different molecules and transfer energy in the form of heat to different layers of the skin. Of course, radio waves simply don't "notice" skin chromophores, so they are safe and effective equally for dark and fair skin. It is also important that the nerve endings are not sensitive to radio waves, so their effect is painless. In this aspect, radio wave technology is a good alternative to laser skin rejuvenation techniques.

The idea of ​​fractional radio wave damage to the skin in order to stimulate its structural rejuvenation was borrowed from: microscopic areas of the epidermis and dermis undergo thermal ablation / coagulation at points spaced at the same distance from each other, and intact skin surrounding the damaged “points” serves as a source of cells and building material used to restore and build a new tissue.

The technology of fractional radiofrequency rejuvenation was first implemented on eMatrix devices (Syneron, Israel) with a nozzle (applicator). On the working surface of the applicator there are parallel rows of bipolar electrodes with a diameter of 200 µm, resembling pins (Fig. 1 and 2). High-frequency energy, the level of which reaches 40 J, is applied to dry skin through each pair of positively and negatively charged electrodes. In the skin, the energy of the alternating electric field is converted into thermal energy, which damages the skin directly under the electrodes pointwise (ie, fractionally). There are areas of healthy skin between the electrodes, and the ablative damages themselves are minimal and in total make up no more than 5-10% of the treated skin area.

Adjustable modes of exposure to the applicator

In aesthetic medicine, for several years now, the method of radiofrequency non-ablative lifting has been used, achieved by volumetric heating of the deep layers of the dermis. Matrix RF is the first RF applicator (no laser or light energy) capable of ablation/coagulation/necrosis and resurfacing of the skin. Studies have shown that the Matrix RF applicator is able to deliver radiofrequency energy to the skin surface with subsequent fractional ablation of the epidermis, and is also able to cause coagulation / necrosis or subnecrosis of the dermis with minimal damage to adjacent skin areas, due to which the recovery is faster and “better”. Schematically, the effect of the Matrix RF applicator on the skin is shown in Fig. 3

On the path of the radio frequency, the skin tissue is not uniform in structure and level of hydration, because of this, different effects occur at different depths and between the electrodes themselves. Due to the fact that the impedance (resistance) of the dry stratum corneum of the skin is higher, high-frequency energy passes through the electrodes themselves (as with monopolar exposure), causing, first of all, ablation of the stratum corneum. And in the deeper layers of the skin tissue (the lower layers of the epidermis and in the dermis), the impedance (resistance) is lower, since the tissue is saturated with water and electrolytes. Therefore, here the RF energy passes between the electrodes (as with bipolar exposure), creating a wider diffuse effect in the form of coagulation / necrosis with little stratum corneum ablation. Due to energy absorption and dissipation, areas of irreversible high frequency effect (ie, ablation or coagulation/necrosis) are surrounded by areas of tissue that have undergone only partial coagulation/necrosis, subnecrosis, or thermal stimulation. In addition, in the deeper layers of the skin there is a zone of temporary thermal exposure, but in it the density of high-frequency energy does not reach the level necessary for the occurrence of other biological effects.

The device has an adjustable action, since it is possible to control the ratio of ablation / coagulation / necrosis effects, the depth of the ablation zones and the limits of coagulation of the surrounding tissue in accordance with the goals of the procedure, medical indications and the characteristics of the treated skin areas. The level of delivered energy varies depending on the selected program (three-level programs) - program A (less aggressive impact), program B and C (more aggressive) (table 1).

In addition, there are two nozzles that differ from each other in the density of the electrodes:

1) standard coating with 64 electrodes (electrode density - 5%);

2) high-density coating with 144 electrodes (electrode density - 12.5%). The "microtunnels" remaining after radiofrequency exposure have the shape of an "inverted funnel" with a wide base facing the dermis (in contrast to the "microtunnels" in the form of a column after laser exposure) (Fig. 5). Thus, RF energy pulses create pyramidal thermal zones, which result in minimal damage to the stratum corneum (i.e., minimal ablation) and maximum heating in the deep layers of the skin (coagulation, warming). As a result of this, the surface of the skin is “pushed out” (raised) upward, and its surface becomes more even. This feature of sublative rejuvenation, along with simultaneous skin contraction, resembles a uniform ablation procedure, but unlike it, it is less traumatic and with a shorter recovery period.

Each of the three programs offers different levels of ablation and skin renewal, with different pulse intensities. When choosing program A (low intensity), B (medium intensity) or C (high intensity), make sure that the skin is completely dry throughout the treatment area.

The Matrix RF setting allows you to automatically select one of three programs for different levels of ablation and skin renewal, differing in pulse intensity: A (low intensity), B (medium intensity) or C (high intensity) (Fig. 6) . The wide capabilities of the device (the presence of three programs) allow, taking into account the individual characteristics of each patient, to increase the effectiveness of treatment and minimize risks.

Features of the procedure

Let's give the floor to doctors who already have experience with the Matrix RF applicator.

David J. Goldberg, M.D.: “I prefer to perform procedures without anesthesia, but sometimes when working with more aggressive parameters, I use local anesthesia. The leather must be thoroughly cleansed, either with alcohol or other degreasing agents, to remove excess oil and remove any residual impurities. You don't have to watch all the time like you do with lasers, and we don't close our patients' eyes so they don't get claustrophobic. Tissue changes during fractional radiofrequency exposure

As already mentioned, the effect of radiofrequency sublation is achieved through a combination of fractional ablation, coagulation and heating of the skin. The RF current propagates within the dermal layer, causing additional contraction of its fibrous structures and neocollagenesis, while the epidermis is minimally affected. Histological analysis performed immediately after the procedure revealed clearly defined areas of ablation/coagulation/necrosis and subnecrosis up to 450 µm deep. Moreover, a regulated relationship between ablation and tissue coagulation/necrosis has been observed: at low energy levels (10–30 mJ), coagulation/necrosis will be the main effect, and at higher energy levels (50–62 mJ) ablation.

On fig. 7 shows histological tissue changes at various times after the procedure. 6 hours after exposure, partial re-epithelialization of the epidermis with a pronounced inflammatory reaction is noticeable in the skin. Moist scabs begin to form 12 hours after the procedure and re-epithelialization continues. Attention is drawn to the appearance of compact and dense collagen fibers and infiltration of leukocytes. 24 hours after the procedure, re-epithelialization of the epidermis is completely completed, and a necrotic crust appears on the surface of the skin. The great advantage of this technology is its independence from the presence of chromophores in the skin, so it can be used for any skin phototype. The risk of side effects, primarily post-inflammatory hyperpigmentation, is also significantly reduced, which is another important advantage of the method.

Clinical effectiveness of fractional radiofrequency rejuvenation

According to the doctor of aesthetic medicine Amy F. Taub (M.D.), “sublative rejuvenation is excellent for correcting structural changes in skin tissue, wrinkles and age-related symptoms. There are practically no risks of adverse reactions and complications, and the recovery period is short. eMatrix is ​​also great for treating scars even on dark skin and I haven't seen a single case of hyperpigmentation after the procedure. Areas that are difficult to treat, such as the periorbital and perioral zones, as well as the neck area, respond well to treatment. This technique can be applied locally and does not result in a demarcation line, so it is not necessary to treat the entire face. Sublative rejuvenation is well suited for the correction of enlarged pores and superficial wrinkles. In fact, the procedure gives some improvement in very deep wrinkles, such as glabellar wrinkles, forehead wrinkles, nasolabial folds.

I agree with this opinion Mark Taylor (M.D., F.A.A.D.):“Sublative rejuvenation is well suited for the correction of such difficult-to-correct aesthetic defects as stretch marks, scars with a corrugated (uneven) structure, scarring of the skin after acne, and also in cases where patients are not ready for a long recovery period and extensive skin damage. Aging skin with deviated texture, photodamage symptoms, and a wrinkled surface responds well to this procedure, and patients are satisfied with the results.”

Michael Gold shares his clinical observations: "Sublative rejuvenation effective in the correction of wrinkles, skin tightening, post-acne and post-traumatic scars; scars can be treated faster than other methods allow. According to my observations, scars can be effectively corrected in 1 or 2 sessions and with a shorter recovery period than with CO2 laser ablation. In this procedure, the risks of post-procedural pigmentation are much lower, and this circumstance is important, because the majority of the population in the world has a dark skin color. A large-scale clinical observation of the effectiveness of fractional radiofrequency rejuvenation was carried out on 67 patients (64 women and 3 men), including 50 Caucasians aged 25 to 67 years. The majority of patients (45.3%) had skin phototype III. The level of elastosis ranged from 2 to 6, in the majority (51%) - 3 and 4. The main problems that patients treated were wrinkles (37.4%) and loose skin (20.5%).

Patients underwent 3 procedures for the entire face area with an interval of 4 weeks; the evaluation of the results was carried out during the 3rd procedure and 6 weeks after the completion of the course. When examining patients, various indicators characterizing the condition of the skin were taken into account. In addition, assessments were made on standardized digital photographs taken before and after the course.

Evaluation of the effectiveness of the procedure, set by doctors, is shown in Fig. 8. All studied parameters (wrinkles, radiance and thickening of the skin, pore size, color), as well as general indicators, prove a significant improvement in the skin both at the third week of observation and 6 weeks after the end of the course (P<0,001). Conclusion

Fractional radiofrequency skin rejuvenation, implemented in the eMatrix device with a Matrix RF nozzle (Syneron, Israel), is a new step in the evolution of physiotherapeutic methods of skin revitalization, which differs from previous technologies in a shorter recovery period, the absence of hyperpigmentation and the possibility of safe use on skin with various types of pigmentation.

The Fractora handpiece (BodyTite device) is designed for a minimally invasive procedure in the face, neck, décolleté; as a result of exposure, fractional radiofrequency resurfacing, ablation, coagulation and subnecrotic heating of tissues occur.   • Before the procedure, a local anesthetic (for example, EMLA 5-18%) will be applied to clean skin 30-40 minutes before the procedure. Sometimes it may be necessary to use tuminescent anesthesia or nerve block.
  • Before the procedure, the face is cleansed with a tonic and an alcohol wipe.
  • After selecting the power parameters and the type of nozzle, the specialist proceeds to the treatment, the procedure time will depend on the treatment area 10-20 minutes.

Radiofrequency lifting has an effective effect on the deep layers of the skin precisely due to the peculiarities of the procedure using radio waves. Modern devices allow you to carefully monitor the patient's condition during the procedure. In particular, the heating temperature of the applicator tip is controlled to ensure that the skin does not overheat and burn. The condition of the subcutaneous fat is also controlled - this becomes possible due to the directed action of the high-frequency electric field.
